Application
Access to the program is through a referral by a health care provider.
The individual must be experiencing:
- mental health problems/illness complicated by age-related dementia
- behavioural problems associated with dementia and/or other severe mental illness
- must be medically stable
- reside in the community which includes living at home, supportive housing or in a facility-based environment such as a long-term care facility.
Once a referral is received, a phone call within two weeks will be made to the referring source.

Description of Services:
Individuals served in this program are generally seniors over the age of 65 or younger persons who have mental health problems or illnesses. Services are also provided to people in long-term care facilities who have behavioural problems related to dementia or to mental illness.
An assessment is completed by a mental health nurse followed by a psychiatric consultation with a psychiatrist who then provides the family physician or other health care provider with diagnosis and treatment recommendations. Once the assessment and psychiatric consultation have been completed, the treatment recommendations will be followed through by the referring health care provider. Referrals to other supports will also be made. Limited follow-up for people with age-related mental illness. This is not a crisis service and individuals must be medically stable.
