Description of Services:
Partnership between London InterCommunity Health Centre and Regional HIV/AIDS Connection * program designed for persons at high risk for Hepatitis C infection due to drug use and those who are Hepatitis C positive who have complex addiction and/or social issues * outreach worker, peer support worker, nurses and social worker support all aspects of clients life - physical, emotional, psychological and spiritual * referrals to other community agencies * screening, prevention, care and treatment of Hepatitis C * dispenses free naloxone take-home kits - lifesaving medication that can restore breathing of a person who has overdosed on heroin or prescription opioid medication
