Application

Primary Care Program provides primary care services to clients of CMHA who do not have access to a family physician and who live in Timmins. Referral is made by the intake or assigned mental health clinician. Referrals of clients from other mental health service providers, and primary care practitioners will also be accepted. Timmins and District Psychiatry can refer individuals who do not have a family physician and require primary care.

Description of Services:

Primary health care refers to an approach to health and a spectrum of services beyond the traditional health care system. Primary care is the element within primary health care that focuses on health care services, including health promotion, illness and injury prevention, and the diagnosis and treatment of illness and injury. The Primary Care Program strives to provide comprehensive primary health care to respond to the needs of the whole person and ensure continuity of care, acute and chronic disease management, as well as health promotion and disease prevention. Offering primary care services within a community mental health organization is a new approach to provide accessible and client-centered care to people with mental health issues. The Nurse Practitioner with the Primary Care Program works collaboratively with mental health service providers, psychiatrists, and physicians in areas of assessment, diagnosis, and prescription of medications, treatment and health promotion to meet the health care needs of clients.
